If you have commercial insurance such as Anthem, Aetna, UHC, MMOH, etc., fill out: New Patient Registration and Records Release.
If you have Bureau of Workers Compensation fill out: New Patient Registration, Records Release, and Workers Compensation Patients.
If you were in an auto accident or other personal injury fill out: New Patient Registration, Records Release, and Personal Injury.
All new patients must fill out this packet.
All new patients must fill out a records release for every doctor seen within the last 18 months.
All new patients who have BWC claims must fill out this form.
All auto accidents and personal injury must fill out this form.
Failing to fill out this paperwork prior to your visit may increase your wait time to see the doctor up to 30 minutes.
